Privacy Policy

This Privacy Policy explains how Fort Worth Family and Implant Dentistry ("we," "us," or "our") collects, uses, and protects information when you visit ftwimplants.com (the "Site") or contact us about our services. It applies to this website only. Information you provide when you become a patient or receive dental care is protected separately under HIPAA and our Notice of Privacy Practices, which we provide at the office.
Information we collect
Information you give us
If you request an appointment, use a contact or virtual-consult form, call, or email us, we collect the information you choose to provide — typically your name, phone number, email address, and any details you share about the care you are seeking. If you book an appointment through our online scheduler, we collect your name, date of birth, mobile number, and email in order to create your appointment in our practice management system; that information is handled as protected health information under HIPAA and our Notice of Privacy Practices. Some booking links may instead use our scheduling provider (Modento) on a separate site (book.modento.io), subject to its own privacy practices.
If you send Dr. Baker photos through the "Send Dr. Baker a photo" form, the photos, your contact details and your note are transmitted over an encrypted connection to a server the practice operates in its Fort Worth office, not to a third-party service, and are handled as protected health information. Photos are re-saved without device metadata (such as location). Dr. Baker's reply is delivered by a private link sent to the mobile number and email you give us; the text and email themselves contain no details about your teeth. Photos and any reply video are deleted 180 days after the reply unless you become a patient of the practice; the record of the consult is kept as part of your file.
Information collected automatically
Like most websites, we and our service providers automatically collect certain technical information through cookies and similar technologies, such as your IP address, device and browser type, pages viewed, referring website, and how you interact with the Site. We use this to operate the Site, understand what content is useful, and measure and improve our advertising.
Cookies, analytics, and advertising
We use Google Tag Manager to manage the following tools on our Site:
- Google Analytics (GA4) — to understand how visitors use the Site.
- Google Ads — to measure the performance of our ads and to show relevant ads to people who have visited the Site (remarketing).
- Meta (Facebook/Instagram) Pixel — to measure and improve the performance of ads we run on Meta platforms, including showing relevant ads to prior visitors.
These tools may set cookies and collect identifiers such as your IP address and cookie IDs. We do not use them to collect sensitive health information from this Site. You can control cookies through your browser settings, opt out of Google Analytics with the Google Analytics Opt-out Browser Add-on, and manage ad personalization at Google My Ad Center and in your Meta ad preferences.
How we use information

- To measure and improve our marketing and advertising.
- To comply with legal obligations.
How we share information
We do not sell your personal information. We share information only with service providers that help us run the Site and our marketing (such as our hosting, scheduling, analytics, and advertising providers named above), and only as needed to perform those services, or when required by law. We do not trade, rent, or license your information, and we do not hand it to data brokers or lead resellers.
Your phone number and text messages
No mobile information will be sold or shared with third parties or affiliates for marketing or promotional purposes. Your mobile number, and your consent to receive text messages, are used only so that this practice can contact you about your own care — booking, confirmations, reminders, and post-operative instructions.
Text-message consent is never shared onward, and it is never a condition of treatment. If you ask us to text you, that permission stays with this practice. We use a messaging carrier to deliver the messages, which necessarily processes the number in order to send them; it is not permitted to use your number for its own purposes. You can stop at any time by replying STOP, and it will not affect your care. See our Terms of Service for the full messaging terms.
Because text messages are not a secure channel, we keep clinical detail out of them — we will not describe a diagnosis, procedure, or symptom by text.
Your choices
You can decline or delete cookies in your browser, use the opt-outs described above, and ask us to update or delete contact information you have submitted by emailing [email protected]. Depending on where you live, you may have additional rights over your personal information; contact us to exercise them.
Children's privacy
The Site is intended for a general audience and is not directed to children under 13. We do not knowingly collect personal information from children under 13 through the Site.
Changes to this policy
We may update this Privacy Policy from time to time. Changes take effect when we post the updated version here with a new effective date.
